This article explains how to log referrals to external agencies. This feature is currently ONLY available for the CHLS and C-Work programs.
1. Navigate to the "Outbound Referrals" object located in the Related List Quick Link section of your case record (exact placement varies by program). Click on the link and select "NEW" to log a new referral. Note that each referral will need to be logged separately.
2. Select "Outbound Referral" from the list of record type options. You'll be directed to a new referral page.
3. Enter the relevant information related to the referral and click SAVE. Key fields include:
- Referral Date: The date you provided the referral to the client.
- Referral Status:
- Referred: Indicates that a referral has been provided to the client and the outcome is not yet known. This is the option you will select for most referrals unless you know that the client successfully or unsuccessfully connected with the referral agency.
- Successful: Indicates that the client connected with the referral agency and is receiving services.
- Unsuccessful: Indicates that the client was not successful in securing services from the referral agency.
- Rejected: Indicates that the client opted not to pursue the referral.
- Referring from Program: Search from the list of programs and find the name of your program.
- Referring from Staff: Enter your first and last name.
- Referring to Organization: Enter the name of the organization/agency where you are referring the client.
- Referral Notes: Enter any notes or additional details about the referral
4. Once the referral has been logged, you can click on the link to edit or update, if needed.
