Request Step
- CM discusses with supervisor
- CM creates and submits Request form in Salesforce
- Fill out Program, Client, Request Amount, household, First Approver, Second Approver(if applicable) etc.
- Request status is set to “Submitted”
Approval and Disbursement Step
- Based on Request amount and Program, email notification will be routed to the PD. if Requested Amount >=250 will also notify leadership PD reports to (Jarekhye, Heather, Hayden, Susan, or Erica)
- PD clicks link in email and then will be directed to the Request record page in Salesforce
- PD fills out Approver field
- PD fills out Approval Date field
- Program then purchase the items family need and then submit receipt to Finance.
Getting the Funds
1. Send a regular check request to finance with all relevant documentation
- Proof of amount owed or invoice
- W-9
2. Include a screenshot from salesforce showing transaction approved
