IV. Childcare Tracker
- Use the Childcare Tracker to log childcare placements. You will need to enter a new childcare tracker for each placement, for each child.
- Childcare End Reason - When a childcare placement ends, you will need to update the childcare tracker and indicate the reason for childcare end. See pages 8-9 for a list of end reasons and definitions.
VI. Assessments
Family Assessment Matrix: The Compass Family Assessment Matrix or “FAM” is a tool that case managers use to track their client’s level of stability on 14 different categories over time. Case Managers at CCSS are responsible for conducting a FAM assessment every 90 days.
VII. Tracking Living Situation (all CoDa users)
CCSS staff are required to track the living situation of clients who enroll in the program. If the client is not enrolled for any reason, housing trackers are NOT required.
Living situation is tracked using the HOUSING TRACKER. There are three different housing stages:
- Intake (Required): Refers to the client's living situation at intake.
- Enrolled: Refers to the client's living situation IF they move while enrolled with CCSS. This housing tracker is NOT required if the client does not move while in the program.
- Program Exit (Required): The family's living situation when they exit CCSS. If the client's living situation has not changed since the Intake (or Enrolled) housing tracker, you will still need to enter a separate Program Exit Housing Tracker. In this case, you can clone the previous housing tracker.

VIII. Tracking Family Income
CCSS Case Managers are required to collect client income on a quarterly basis (and update income if it changes during the quarter). All earned income, cash, and non-case benefits are tracked on the Income Tracker. For instructions on how to enter income into CoDa, visit the following links.
